From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On 02/03/2021 at approximately 1400L time, N3215Z, a 1959 PA-22 Piper tri-pacer, serial number 22-7155 privacy data omitted departed the prepared surface during the landing rollout on rwy 30 (4,199 x 72 asphalt) at Defiance Memorial (kdfi), Defiance, OH. The winds recorded at kdfi per the plymouth state weather database were 280/7. This resulted in a left crosswind component of 2.4 KTS and headwind component of 6.6 KTS. The landing at kdfi was the final leg of a VFR flight that originally departed from privacy data omitted airport (4OH8), a private grass runway 1800 x 100. The PIC privacy data omittedthen landed at the williams county airport (OG6), in bryan, OH, where he picked up (1) passenger. During the ground operations at OG6 the aircraft was taxied through some patches of snow and other surface contaminants. The aircraft then departed and flew for approximately 30 to 40 minutes prior to the landing at Defiance Memorial (kdfi). Upon landing the PIC reported that he lacked any lateral control and the aircraft subsequently departed the prepared surface to the left, striking a snow bank and the taxiway c sign before coming to rest nose low, upright, in the snow covered grass. No injuries were sustained by the pilot or passenger.

Photos taken by airport personnel confirm that the kdfi runway surface was dry and contaminant free, they also show skid marks from what appears to be the left tire for approximately 700-900 feet prior to the point the aircraft departed the runway. On 02/10/2021 sent these photos to NTSB investigator privacy data omitted held a phone conversation on 02/10/2021 with privacy data omitted, and based on the images and his interview of the PIC he determined that this would not be categorized as an accident based on the damage sustained by the aircraft. On 02/11/2021 at 0900L, had a phone conversation with the PIC, privacy data omitted where he described the events of the flight and shared his opinion that he believed the contamination picked up during ground operations at the williams county airport (OG6) might have frozen in the left brake during the flight time prior to landing at Defiance (kdfi), resulting in the lack of directional control on rollout. On 02/10/2021 at 1050L had a phone conversation with the Defiance Memorial Airport manager, privacy data omitted where he confirmed that the left tire of N3215Z was found flat, he could not confirm that it occurred during the landing or after departing the runway yet it was congruent with the skid marks on the runway, and the right tire had remained inflated. Further confirmed with privacy data omitted that he had received a quote to repair the damage to property, the taxiway c sign, for less than $3,000. Also verified that there was a NOTAM posted to document the missing taxiway c sign. Ensured all parties had the FSDO contact information if any further issues arose.

The NTSB intake form contained the following call details: ?pilot reported: around 2 pm EST, 2/3, Defiance, OH. Piper PA-22 (N3215Z) veered left off runway 30 during landing and struck a snow bank and taxiway sign. 2 POB, no injuries. Damages: left landing gear collapsed, right landing gear is slightly damaged, left wing lift strut, nose bowl, engine cowl, propeller.?

As the FAA filed it

ON 02/03/2021 AT APPROXIMATELY 1400L TIME, N3215Z, A 1959 PA-22 PIPER TRI-PACER, SERIAL NUMBER 22-7155 ^PRIVACY DATA OMITTED^ DEPARTED THE PREPARED SURFACE DURING THE LANDING ROLLOUT ON RWY 30 (4,199 X 72 ASPHALT) AT DEFIANCE MEMORIAL (KDFI), DEFIANCE, OH. THE WINDS RECORDED AT KDFI PER THE PLYMOUTH STATE WEATHER DATABASE WERE 280/7. THIS RESULTED IN A LEFT CROSSWIND COMPONENT OF 2.4 KTS AND HEADWIND COMPONENT OF 6.6 KTS. THE LANDING AT KDFI WAS THE FINAL LEG OF A VFR FLIGHT THAT ORIGINALLY DEPARTED FROM ^PRIVACY DATA OMITTED^ AIRPORT (4OH8), A PRIVATE GRASS RUNWAY 1800 X 100. THE PIC ^PRIVACY DATA OMITTED^THEN LANDED AT THE WILLIAMS COUNTY AIRPORT (OG6), IN BRYAN, OH, WHERE HE PICKED UP (1) PASSENGER.

ON 02/10/2021 SENT THESE PHOTOS TO NTSB INVESTIGATOR ^PRIVACY DATA OMITTED^ HELD A PHONE CONVERSATION ON 02/10/2021 WITH ^PRIVACY DATA OMITTED^, AND BASED ON THE IMAGES AND HIS INTERVIEW OF THE PIC HE DETERMINED THAT THIS WOULD NOT BE CATEGORIZED AS AN ACCIDENT BASED ON THE DAMAGE SUSTAINED BY THE AIRCRAFT. ON 02/11/2021 AT 0900L, HAD A PHONE CONVERSATION WITH THE PIC, ^PRIVACY DATA OMITTED^ WHERE HE DESCRIBED THE EVENTS OF THE FLIGHT AND SHARED HIS OPINION THAT HE BELIEVED THE CONTAMINATION PICKED UP DURING GROUND OPERATIONS AT THE WILLIAMS COUNTY AIRPORT (OG6) MIGHT HAVE FROZEN IN THE LEFT BRAKE DURING THE FLIGHT TIME PRIOR TO LANDING AT DEFIANCE (KDFI), RESULTING IN THE LACK OF DIRECTIONAL CONTROL ON ROLLOUT. ON 02/10/2021 AT 1050L HAD A PHONE CONVERSATION WITH THE DEFIANCE MEMORIAL AIRPORT MANAGER, ^PRIVACY DATA OMITTED^ WHERE HE CONFIRMED THAT THE LEFT TIRE OF N3215Z WAS FOUND FLAT, HE COULD NOT CONFIRM THAT IT OCCURRED DURING THE LANDING OR AFTER DEPARTING THE RUNWAY YET IT WAS CONGRUENT WITH THE SKID MARKS ON THE RUNWAY, AND THE RIGHT TIRE HAD REMAINED INFLATED. FURTHER CONFIRMED WITH ^PRIVACY DATA OMITTED^ THAT HE HAD RECEIVED A QUOTE TO REPAIR THE DAMAGE TO PROPERTY, THE TAXIWAY C SIGN, FOR LESS THAN $3,000.
