From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Pilot was on fourth load of the day and was on roll out on departure and could not get the aircraft tail wheel up enough to get the lift needed for take off due to gusts of wind. The pilot states that he is usually on the "step" and crossing a ditch that runs east to west. Shortly before reaching this point the nose of aircraft was veering to the left and pilot applied right brake, but had little to no effect, and he was at the point of no return. The pilot states that due to a lack of flying speed, his only option was to try and jump the ditch. In doing so the main gears hit the north embankment and had departed from the airframe. The aircraft skid and came to a stop. The pilot states that he exited the aircraft and started walking to his truck and while walking he called privacy data omitted AWOS and they reported gusts of 20 knots from the south/southwest. (Quartering tailwind). By evidence at the crash site it appeared the pilot was on take off and experienced a side gust of wind and he veered off to the left of the runway and hit the ditch with his left and right MLG, breaking off both gears, causing the left wing to impact the ground, the prop striking the ground and the aircraft sliding on the belly until its final resting place on the side of an embankment to the left of the runway.

As the FAA filed it

THE PILOT STATES THAT HE EXITED THE AIRCRAFT AND STARTED WALKING TO HIS TRUCK AND WHILE WALKING HE CALLED ^PRIVACY DATA OMITTED^ AWOS AND THEY REPORTED GUSTS OF 20 KNOTS FROM THE SOUTH/SOUTHWEST.
