From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

During takeoff the airplane experienced a left tire blow out during rotation. The airplane veered left while the pilot applied right braking with no effect. The airplane departed the runway and hit a small drainage ditch causing the right landing gear to break off and the nose to impact the ground. The left tire appeared to have been cut from the inboard sidewall perpendicular to the first tread groove. Part of the tire tube extended from the cut tire and appeared to be cut as well. The right wing impacted the ground causing deflection in the wing spar noted by buckling of the upper wing skin. The right aileron was damaged when the inboard edge impacted the flap outboard edge. Damage to the five bladed prop was consistent with the engine being under power and all five blades bowed back from the hub. After the engine bay paneling was removed, the upper aft engine truss tube was observed to be bent and the turbo-prop engine buckled in two places. Although the aircraft was substantially damaged, the pilot was not injured. Review of the airframe maintenance records showed an entry by the pilotprivacy data omittedthat he replaced both main tires (air hawk 29X11.0-10 4CB, pn 31034), 02/23/2021.

As the FAA filed it

REVIEW OF THE AIRFRAME MAINTENANCE RECORDS SHOWED AN ENTRY BY THE PILOT^PRIVACY DATA OMITTED^THAT HE REPLACED BOTH MAIN TIRES (AIR HAWK 29X11.0-10 4CB, PN 31034), 02/23/2021.
