From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

May 20, 2021 at 10:30AM local time thrush N4090W departed 1F5 airport with only the pilot on board in order to spray a field approximately 15 miles NE of the Hoxie-Sheridan County Airport. The pilot verified he had 100 gals of fuel before takeoff. After applying the load of fungicide to the intended field, he was heading back to the airport when the engine failed. The aircraft was at 750 AGL and the pilot had to point the aircraft at the ground to keep airspeed above stall. He flipped on the fuel boost pump and igniters but the engine power was not restored. An emergency landing field was quickly chosen, a pasture, which had sustained recent heavy, rains. He landed with full flaps and was initially successful in the landing however, before the pilot could get it completely stopped the aircraft rolled off a cutbank coming to rest upright. The total flight duration from takeoff to landing including the aerial application was 45 minutes.

After the aircraft was relocated to a hanger for inspection, all residual fuel was removed from the wing tanks and header tank. Approximately 3 gallons of fuel was recovered which equates to the unusable fuel usually held in the tanks. The fuel did not appear to be contaminated and the fuel screen in the firewall was relatively clean.

As the FAA filed it
