From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On June 10, 2021, at approximately 2028 eastern daylight time (EDT), a Cessna CE-150H, N22658 (de-registered), crashed in a bean field approximately 4 miles south-southeast of Muncie, Indiana. The non-rated pilot, the sole occupant, suffered serious injuries when the aircraft struck the ground. The aircraft was operated by the privacy data omitted under the provisions of title 14 code of federal regulations part 91. Visual meteorological conditions prevailed and no flight plan was filed. The flight departed from an unimproved grass strip immediately before the accident.

A witness discussed the high density altitude (high temperature and high humidity) with the pilot prior to the takeoff and asked him not to fly. Shortly after the aircraft became airborne, the witness saw the aircraft stall, then spin, striking the muddy field with the left wingtip. The aircraft cartwheeled and the pilot was partially ejected from the aircraft. A review of the weather did confirm a high density altitude at the time of the crash.

As the FAA filed it

THE AIRCRAFT WAS OPERATED BY THE ^PRIVACY DATA OMITTED^ UNDER THE PROVISIONS OF TITLE 14 CODE OF FEDERAL REGULATIONS PART 91.
