From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Who: perez lundqvist, johan hans (instructor giving training), hurt, farilyn laniece (pilot flying)

When: 07/18/2021 20:15Z

Where: Chicago Executive Airport, Wheeling, IL (pwk)

What: the pilot flying, a certified flight instructor (CFI) obtained instruction for a 14 CFR part 61 flight training provider for the purpose of getting back into flying after a 3+ month long break from flying. The pilot flying had been flying low wing aircraft, and was transitioning to high wing aircraft. The instructor assigned was to work with the pilot flying on currency, and to working towards the cfi-instrument. The flight originated at pwk with multiple landings at campbell airport (C81), before returning to pwk. Pwk was landing runway 16 with winds of 060 degrees at 9 knots creating a crosswind from the left. The pilot described rounding out and into the flare and aligning the nose of the aircraft with the runway, but with improper aileron correction and touched down with the right wheel first. The left wing raised with the wind and the aircraft was blown to the right side of the runway. The flying pilot attempted to correct with left rudder, and insufficient aileron and exacerbated the left wings ascent. The right wing struck the runway surface, and the aircraft veered into the grass to the left of runway 16. The aircraft stopped in the grass and the occupants evacuated. No injuries to the flying pilot and the instructor. Aircraft was removed from the grass by the airport authority.

Causal and/or contributing factors include:

The flying pilot was flying low wing aircraft for most of the training received, and had not flown a high wing in years prior to the incident. The flying pilot had taken 3 months off of flying, and was not current before the incident. The flying pilot used a crosswind technique which inputs the wind corrections at the last few moment prior to touchdown. While not incorrect, this methods gives the instructor giving training less time to react to improper corrections. The instructor giving training was flying from the left seat, when normally occupying the right seat leading to some parallax issues.

Identified hazards and/or ineffective risk controls include:

The pilot flying did not touchdown with the windward main landing gear, and used incorrect aileron deflection for the crosswind. The instructor giving training was slow to react to rectify the pilot flying?s incorrect crosswind correction.

Recommendations for systemic corrective action to reduce risk of future occurrence;

The flying pilot has received instruction in crosswind technique, and how to teach cross wind landings to future students. Counseling has been conducted on how to perform and teach crosswind landings in the future. The instructor giving training was counseled on complacency and how to identify, and remedy situations where complacency may occur.

Comments: both the flying pilot and the instructor giving training were compliant throughout the investigation. This case is considered closed with compliance action counseling.

As the FAA filed it

RECOMMENDATIONS FOR SYSTEMIC CORRECTIVE ACTION TO REDUCE RISK OF FUTURE OCCURRENCE;
