What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
Investigation: I was notified by Fullerton Airport air traffic control that on August 2, 2021 at the Fullerton Airport runway 24 at approximately 1420 local time N6115V lost power, clipped a runway light and landed at the very threshold of runway 24. The FAA became aware on August 5, 2021.
Root cause analysis (rca): this nonconformance was due to failure to use the landing checklist and adjust the mixture prior to landing. The pilot was given a straight in clearance to land but had to lose altitude while doing so. Once on glide-path the pilot went to advance power but the throttle did not respond. The aircraft got low on the glide-path and clipped a lead-in stanchion light prior to touchdown at the beginning of runway 24. The aircraft was able to exit the runway prior to stopping. The pilot observed that the mixture was still full out (lean) which was the cause of the loss of engine power. The pilot was able to restart the engine with no issues.
Corrective action: realized his failure to use the checklist was directly responsible for the aircraft?s loss of power. Stated that he always used the ?
(Gump) checklist but never really got to an approach position as he was given a constant descent from 11,000 feet MSL down to the runway. Assured me that from now on he would use his checklist on every flight.
Comments: I used counseling to ensure compliance to operations]. I am satisfied the pilot?s corrective actions fully address the identified cause for continued safe operations. I recommend no further action or additional follow-up.
As the FAA filed it
INVESTIGATION: I WAS NOTIFIED BY FULLERTON AIRPORT AIR TRAFFIC CONTROL THAT ON AUGUST 2, 2021 AT THE FULLERTON AIRPORT RUNWAY 24 AT APPROXIMATELY 1420 LOCAL TIME N6115V LOST POWER, CLIPPED A RUNWAY LIGHT AND LANDED AT THE VERY THRESHOLD OF RUNWAY 24.
