From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Security video showed the accident aircraft taxied to runway 02. Landing S-76 helicopter traffic was over the runway and was intending to hover taxi clear when the pilot began to taxi onto the runway and takeoff. The aircraft became airborne behind the helicopter and climbed 20-30 feet before appearing to roll slightly to the right followed by a roll to the left going inverted. The aircraft struck the runway inverted and slid about 30 feet coming to rest near the right edge of the runway. The aircraft began to burn almost immediately and was destroyed by the fire. The single occupant pilot was fatally wounded.
