From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

1. Description: on 30 November 2021, privacy data omitted, registration number c-gjzv, a Canadair crj aircraft, taxied for takeoff at San Diego international airport (ksan) with a destination of vancouver (cyvr). The aircraft had 69 passengers and a total of 4 crew members. Because of delays, the aircraft was taxiing during night conditions. The weather report given for pushback and takeoff were the following:

Speci ksan 300156Z 00000KT 4SM br SCT002 16/15 A3003 rmk AO2 vis s-w 1 T01560150=

Speci ksan 300235Z 30004KT 1/2SM R09/1200V2000FT fg BKN002 14/14 A3004 rmk AO2 vis ne-e 2 s-w 1/4 T01440144

After taxiing to B1 for runway (rwy) 27, the crew confirmed the runway number with the sign and pavement markings at the B1 hold short line. B1 is at the beginning of an 1800 foot displaced threshold for rwy 27. When cleared for takeoff, the captain maneuvered the aircraft into position, lineup checks were completed, the horizontal situation indicator (his) was referenced for runway heading, and controls were transferred to the first officer. Neither pilot could see the rwy 27 threshold lights which were 1,800 feet away, and they do not recall the approach lights being on at the time.

Prior to the takeoff roll, the crew had mistakenly lined up with the left runway edge lights on the displaced threshold, believing they were centerline lights. During the takeoff roll, the pilots felt "surface roughness" which was actually the left main gear hitting the left edge lights. They reported thinking that the roughness was from the displaced threshold approach light bar area. As the aircraft rolled down the runway, three edge lights were hit in succession but the crew was not aware. As the aircraft closed with the approach end of the runway, the crew was able to see the centerline lights and corrected right to line up with the lights.

As the FAA filed it

1. DESCRIPTION: ON 30 NOVEMBER 2021, ^PRIVACY DATA OMITTED^, REGISTRATION NUMBER C-GJZV, A CANADAIR CRJ AIRCRAFT, TAXIED FOR TAKEOFF AT SAN DIEGO INTERNATIONAL AIRPORT (KSAN) WITH A DESTINATION OF VANCOUVER (CYVR).
