On 06/02/2022 at approximately 1220 PDT privacy data omitted a Beechcraft m/n 19A, s/n MB-432 was involved in a double fatal accident. On 06/03/2022 myself ASI privacy data omitted and ASI privacy data omitted traveled to kove to perform an on-site examination. The aircraft came to rest past the departure end of runway 13 at coordinates 39 29? 10? N and 121 36? 44? W. the aircraft impacted in a near nose first attitude. The aircraft contacted the ground left wing first which resulted the tail section (aft of baggage compartment) buckling to the left. The right side of the tail section showed a skin shear at the buckle point. The right wing aft spar attachment point at the fuselage was sheared. The fuselage area around the cockpit was fragmented from impact forces. A debris field was observed and extended approximately 15 feet from impact point, most of the debris was plexi-glass from the windscreen and side windows. A portion of the instrument panel was examined and showed the air speed indicator at approximately 50 mph. Photos were taken of the accident site. Nothing at the scene was altered or moved as the NTSB iic was arriving on scene later that day.
An interview was performed with the witness/friend/student pilot with the following statement: one of the instructor pilots on board the accident flight was his instructor and friend. Two weeks prior to the accident flight he was to receive instruction in the aircraft. A pre-flight was performed and a run-up prior to departure with no anomalies. The instructor and student then proceeded onto the runway surface and began the t/o sequence once the aircraft left the ground the student stated that it did not have sufficient power to continue and that the instructor took the controls and landed the aircraft as they still had plenty of runway. A second attempt was made with identical results. The instructor and student taxied back to the hanger where they stowed the aircraft. The accident flight took place 2 weeks after the 2 aborted t/o?s this time with two instructors on board and the student watching and videoing on the ramp. The student stated that the instructors were friends. The student said a preflight was performed and that no fuel was added prior to the accident flight as he and the instructor put fuel in the a/c two weeks ago. Prior to the t/o attempt a run-up was performed and it is presumed it was normal as the two instructors performed the t/o. the aircraft reached approximately 300 ft. AGL and the student could tell there wasn?t enough power to continue, the aircraft then did a stall/spin and impacted the ground. Inspectors privacy data omitted and myself were able to watch the video the student made that showed the accident flight, the video ended as the aircraft stalled. The spin and impact with the ground was not caught on the video.
The butte county sheriff?s department secured the aircraft logbooks from one of the aircraft privacy data omitted the day of the accident. A butte county sheriff brought the logbooks to the airport so we could review them. The logbooks were then given back to the sheriff?s department.
A teardown inspection has been scheduled for July 21, 2022 at a secure facility. If any amendments need made to this report it will be amended at that time.
