From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Pilot in command (PIC) stated he was performing a soft field style departure from runway 35 and had the nose wheel off the ground when the aircraft began moving to the left of the runway centerline. PIC applied right rudder but large rudder input did not correct. PIC aborted takeoff, lowered the nose wheel to the ground and was coming to a stop when the nose landing gear hit a concealed rock and broke the nose gear fork causing the lower engine cowling and propeller to impact the ground. Minor damage to the lower firewall does not constitute substantial.
