From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On August 27, 2022, at approximately 11:30 CDT, privacy data omitted suffered fatal injuries while skydiving at privacy data omitted in Ottawa, IL 61350. A privacy data omitted camera mounted toprivacy data omittedhelmet was recovered and captured the entire event on video. privacy data omitted Jumped from aircraft privacy data omitted without incident at an approximate altitude of 13,700FT, as observed from privacy data omitted? Wrist mounted altimeter. The main parachute opened just after privacy data omitted checks his wrist altimeter at approximately 4,100FT. The aft left slider ring prematurely detached from the slider but the video is unclear as to exactly when or how the slider ring detached. The ring stayed on the lines near the top of the canopy, which appeared to restrict the canopy from fully opening causing a slow left turn. privacy data omitted Reached up the risers toward the slider and eventually removed it. Once the slider was removed and released by privacy data omitted an aggressive left turn began placing him and the canopy 90 degrees perpendicular to the ground. He continued to reach toward the risers until he impacted the ground. There is no evidence from the video or privacy data omittedequipment that an attempt was made to use the cutaway handle to deploy the reserve parachute.

ON AUGUST 27, 2022, AT APPROXIMATELY 11:30 CDT, ^PRIVACY DATA OMITTED^ SUFFERED FATAL INJURIES WHILE SKYDIVING AT ^PRIVACY DATA OMITTED^ IN OTTAWA, IL 61350. A ^PRIVACY DATA OMITTED^ CAMERA MOUNTED TO^PRIVACY DATA OMITTED^HELMET WAS RECOVERED AND CAPTURED THE ENTIRE EVENT ON VIDEO. ^PRIVACY DATA OMITTED^ JUMPED FROM AIRCRAFT ^PRIVACY DATA OMITTED^ WITHOUT INCIDENT AT AN APPROXIMATE ALTITUDE OF 13,700FT, AS OBSERVED FROM ^PRIVACY DATA OMITTED^? WRIST MOUNTED ALTIMETER. THE MAIN PARACHUTE OPENED JUST AFTER ^PRIVACY DATA OMITTED^ CHECKS HIS WRIST ALTIMETER AT APPROXIMATELY 4,100FT. ^PRIVACY DATA OMITTED^ REACHED UP THE RISERS TOWARD THE SLIDER AND EVENTUALLY REMOVED IT. ONCE THE SLIDER WAS REMOVED AND RELEASED BY ^PRIVACY DATA OMITTED^ AN AGGRESSIVE LEFT TURN BEGAN PLACING HIM AND THE CANOPY 90 DEGREES PERPENDICULAR TO THE GROUND. HE CONTINUED TO REACH TOWARD THE RISERS UNTIL HE IMPACTED THE GROUND. THERE IS NO EVIDENCE FROM THE VIDEO OR ^PRIVACY DATA OMITTED^EQUIPMENT THAT AN ATTEMPT WAS MADE TO USE THE CUTAWAY HANDLE TO DEPLOY THE RESERVE PARACHUTE.
