From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

After a normal taxi and run-up, aircraft privacy data omittedwas cleared for take-off on runway 9R at ptk. The sole occupant pilot noticed no shimmy or "roughness" while on the take-off roll. During climb-out at an altitude of approximatley 100 feet, the crew of an aircraft waiting departure radioed the tower that they witnessed the nose wheel depart the aircraft and wheel assembly was on the runway. This was relayed to the pilot and after one low-approach the pilot performed a soft-field landing on runway 9L. During main gear touchdown the pilot cut-off the engine, held the nose of the aircraft off the runway for as long as possible until the aircraft came to a stop. The pilot was uninjured and emergency personnel took several photofgraphs for the FAA and towed the aircraft back to the flight schools hangar.
