The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
During landing touchdown on runway 9L at tmb, aircraft right main gear collapsed and the propeller hit the ground aircraft veered to the right into the grass east of the taxiway g. inspectors from SO19 responded to the accident scene on Tuesday, April 18, 2023, and documented the scene . Photos show N12RX, a Rockwell International 112, in the grass area south of runway 9L and east of taxiway g at tmb. Signatures found are consistent with propellers striking the ground with the engine running in the area leading up to where the aircraft came to a stop. The runway and grass area have marks consistent with the right main gear being collapsed and dragging. During the inspection, streaks were noted (MIL-STD-5606 fluid) in the lower fuselage area back to the tail cone. The leak appeared to be running back towards the tail indicating it occurred in flight, but appeared to be clean indicating it had not been there for long and had not accumulated
On April 18, 2023, the aircraft was recovered and towed to the hangar. Inspectors from SO19 examined the aircraft. Below are what was noted:
Structure:
The right-wing shows deformation of the underlying structure and leading outboard edge, which is considered substantial damage. In addition, extensive damage on the right side of the nose wheel well and doors.
Engines/propellers:
Propellers sustained significant damage during the accident. All three blades were bent and material was lost. This is consistent with the signatures noted at the accident scene and the signatures on the runway surface.
On April 24, 2023, SO19 inspectors performed a gear swing to investigate the gears funcionality and the hydraulic leak. The hydraulic power pack contained very low or almost no fluid, the system was serviced, and the retraction and the extension of all three gears were performed. A significant leak was noted on hydraulic power pack pn HYC5001 sn AH-5707. The power pack was removed and routed for testing at the request of the NTSB. The unit had been installed and aircraft flown 1.5 hours before the failure.
Hydraulic power pack pn HYC5001 sn AH-5707 was sent to manufacturer cox airparts, LLC. Wichita Kansas. To determine the leak's root cause, cox proceeded to bench test the hydraulic power pack and confirmed leakage during the retraction in the lower valve assembly; however, the hydraulic power pack was not repaired or overhauled. Inspectors of the FSDO-19 and NTSB request further action to determine failure of the hydraulic power pack.
During the review of the ramp video at the Miami dade aviation department at tmb airport we verified the collapse of both nose and RH landing gears during the touchdown; this information provides evidence of a failure due to a leak of hydraulic fluid to provide positive locking of the landing gear at touchdown.
As the FAA filed it
DURING LANDING TOUCHDOWN ON RUNWAY 9L AT TMB, AIRCRAFT RIGHT MAIN GEAR COLLAPSED AND THE PROPELLER HIT THE GROUND AIRCRAFT VEERED TO THE RIGHT INTO THE GRASS EAST OF THE TAXIWAY G.
