From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

When: April 24, 2023 at 0435Z

Where: Kankakee, IL (kikk)

On 4/24/2023 at approximately 23:25 CDT, the pilot in command (PIC) of a PA-28R-200 failed to extend the landing gear resulting in a gear up landing. The instrument rated, private pilot on board was receiving flight instruction from a certified flight instructor (CFI) departing eagle creek airpark (keye) and arriving at Greater Kankakee Airport (kikk). The flight was conducted at night, under visual meteorological conditions (VMC) and using visual flight rules (VFR). The nearest automated weather observation station (AWOS) reported calm winds and clear skies. METAR kikk 250435Z auto 13005KT 10SM BKN060 OVC085 09/M01 A3014 rmk AO2 T00881012. The aircraft sustained minor damage and there was no post-crash fire. In addition, there were no injury to persons or property on the ground.

The CFI on the incident flight was providing instruction to an instrument rated, private pilot interested in obtaining his complex endorsement. The private pilot had no previous experience in a complex aircraft. The incident flight was his first training flight. The CFI had experience in complex aircraft, however had not flown a complex aircraft for 9 months prior to the date of the incident flight. The CFI had 1.4 hours in the PA28R-200, which was obtained on a 141 proficiency check conducted just prior to and on the same day as the incident flight.

The incident flight was conducted at night, with the incident occurring at 23:25 CDT. It was the private pilot’s first flight of the day. The CFI had flown several times, totaling 6 hours of flight time. The cfi’s first flight of the day was at 8:00 CDT. The CFI indicated that she was fatigued.

Upon arriving at their destination airport, kikk, the private pilot maneuvered to enter a downwind for runway 22. As he reduced power to descend to traffic pattern altitude, the gear warning horn was audible. The CFI added power to 20 inches of manifold pressure (mp) and stated that it was too early to reduce power as the aircraft likes to sink. The CFI indicated that the private pilot did use the landing checklist on downwind, but skipped the landing gear. The CFI acknowledged that she did not catch the mistake. The private pilot added flaps incrementally, with full flaps extended on final. Neither pilot indicated they were aware of a gear warning indication on final. The aircraft settled on the runway with the gear retracted and the gear handle in the retracted position. The aircraft came to rest in the grass on the right side of runway 22.

As the FAA filed it
