What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
The aircraft was performing an off-airport takeoff from a grass field, the take off field was 700' long and take off direction was to the south in the direction of trees. Takeoff conditions were day, VMC with light winds from the south. Airplane lifted off and the pilot began a right turn in which the right-wing tip grazed the ground. Pilot leveled the airplane, the aircraft settled with the left wheel contacting the ground with a side load great enough to break the left wheel axal, the side loading caused a rolling action causeing the left wing to strike the ground. Aircraft did not roll over and left wing and left wheel axal were damaged.
