From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

Two contributing factors:

1. It was noted that the driver of the lavatory truck was wearing boots that were extremely worn down and smooth on the bottom.

2. privacy data omitted Ground handling manual (egom) section 2.7.2 does not have a specific caution or warning about standing directly behind a vehicle being backed up to an aircraft. It only says they must maintain visual contact with the driver.

Reccomendations:

A reccomendation is being sent to the egom committee that they add a warning to the manual about standing directly behind the vehicle they are guiding.

A reccomendation that a safety risk assessment is done on personal equipment for each team member including footwear.
