From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

The incident occurred during a simulated practice of a single-engine approach to landing. The flight instructor (CFI) reported being task-saturated and focused on radio communications due to high traffic volume in the practice area around the VOR. The student pilot was struggling to control the aircraft during the last instrument approach prior to the gear up landing. Upon reaching the decision altitude, the CFI instructed the student to remove her vision-limiting device and execute a visual approach to land. Both the CFI and student later told the FAA that the before-landing and landing checklists were not completed, which meant that the gear status was not confirmed. As a result, the student proceeded with the landing and performed a gear-up landing.

As the FAA filed it
