What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
On 11/05/2024, at 1638 local, subject aircraft departed runway 22L at kffz enroute to kpvu. The flight was being conducted under 14 CFR part 91 and was occupied by 1 PIC and 4 passengers. The PIC held an apsel and amel with instrument rating and was type rated on CE-510S, HA-420 aircraft. After receiving ATC clearance for takeoff on runway 22L, the aircraft accelerated to V1 takeoff speed before mid-field. Unofficial ADS-B data indicated that the aircraft began decelerating at approximately taxiway D4 and departed the runway traveling approximately 90 KTS. Eyewitness accounts, surveillance camera footage, and landing gear witness marks substantiate that the aircraft never became airborne. After departing the overrun area at approximately 70 KTS, the aircraft crashed through a chain link perimeter fence and impacted a vehicle traveling northbound on greenfield rd. the aircraft and vehicle traveled approximately 30 additional yards and came to rest on the southbound curb of greenfield rd. oriented from tail to nose on a bearing of 273 degrees (magnetic). The aircraft fuselage and fuel cells were compromised and a post-crash fire ensued. One passenger escaped the aircraft, sustained serious injury, and was transported to the hospital. The PIC and 3 passengers were fatally injured. The sole occupant of the vehicle struck by the aircraft was also fatally injured.
On 10/30/2024, the PIC experienced a stall pusher fail and subsequent auto pilot fail while enroute from kaus to kffz at an altitude of approximately 40K MSL. PIC downloaded aircraft data and forwarded to Honda for analysis. Honda analyzed the faults and were able to confirm the presence of the faults, but “no trigger to record the fault going active” could be determined. The fault(s) could not be duplicated on the ground. Honda advised PIC to “monitor and see if it returns.” no maintenance record entries were made to document the event.
The subject aircraft was being maintained under the manufacturer’s recommended maintenance program (HA-420 alim rev. G, 05-20-00). A review of available aircraft records noted that the aircraft had completed a 600-hour inspection on 06/23/2023 at 603.0 total time. All applicable airworthiness directives and service letters were being tracked and noted as current. 91.411/91.413 checks were performed on 03/16/2023 and current.
Details of the examination:
The fuselage had split open immediately forward of the wing leading edge at fs 225. Post crash fire damaged portions of the fuselage, wings, and empennage. The entire interior of the airplane was fire damage and covered in black soot. The tail section remained intact. The speed brake was fully deployed. Both engines remained attached to their respective pylons which remained attached to the wings. The aileron and its trim tab remained attached to the wing. Flight control continuity was established on-scene.
The cockpit had minimal fire damage but was coated in black soot. Both pilot seats remained attached to the airframe. Both control columns were undamaged. The instrument panel remained intact with both pilot and copilot primary flight displays (PFD), the multifunction display (MFD), two garmin touch controllers undamaged. The led master caution/warning lights and panel were in place. Pilot and copilot oxygen masks were in their stowed locations.
All three pitot tubes were examined for obstruction. The left probe was obstructed with dirt (impact related), and the two on the right side were clear. The two ice detectors were attached to the airframe and both exhibited impact damage. No baggage was found within the wreckage. Two cell phones and an ipad were recovered from the cockpit.
Several system components were removed for laboratory examination by the NTSB. Until analysis has been completed, it is not possible to ascertain if FAA areas of responsibility contributed to the accident. This report will be amended accordingly following NTSB analysis.
As the FAA filed it
ON 11/05/2024, AT 1638 LOCAL, SUBJECT AIRCRAFT DEPARTED RUNWAY 22L AT KFFZ ENROUTE TO KPVU.
