The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
privacy data omitted, A B-777-300ER, struck what appeared to be a bird of prey on departure from Washington Dulles airport (iad). The following is the description of the event from the flight crew:
“On rotation from 01R, large bird flying right to left across the runway impacted the aircraft. Almost immediately a very strong burning smell in the flight deck. Reported to the tower that we hit something on departure and copied a phone number to report damage later. We received a cabin call shortly afterward which confirmed the same in the cabin. Aircraft cleaned up and the smell very quickly dissipated without action in the flight deck, entirely dissipating by approximately 10,000FT. Flight crew had initially suspected bird had entered pack inlet and so we asked the crew to monitor the strength of fumes over 5 minutes and report back. Crew reported no improvement over a series of calls lasting approximately 10 minutes, during which time flight deck was entirely clear, so no need for oxygen. Flight crew decided to initiate the smoke, fire, and fumes (sff) checklist in response to cabin situation, with P2 initially running the checklist. Checklist run co point of 'initiate diversion' with no improvement in the cabin. Cabin crew reported that fumes were strongest between D2 and D3. At this point, aircraft was flying past nyc area which was covered in ts, so the decision was made to continue checklist whilst pointing towards bos. Got as FAR as turning off left pack (still no improvement), before stopping sff checklist to prepare for landing in bos. Emergency declared with ATC and radar vectors given to LH downwind for runway 22L. Aircraft configured for F30, abrk 4, max reverse (very slightly overweight) landing. “
Maintenance found a 1.5 inch crack on the outlet guide vane (ogv) fairing and missing material less than ¼ of the panel and no loose material. A detailed borescope inspection was conducted on engine 1 compressor stages 1/2/4/8 in accordance with the aircraft maintenance manual. No damage to the compressor stages was noted and no bird remains were found.

^PRIVACY DATA OMITTED^, A B-777-300ER, STRUCK WHAT APPEARED TO BE A BIRD OF PREY ON DEPARTURE FROM WASHINGTON DULLES AIRPORT (IAD).
