From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Immediately after takeoff, the flight crew received a master warning "ground spoilers fail" eicas message. The crew finished the after takeoff checklist and pulled up the qrh. Qrh only instruction was to land flaps full. The captain opened the synoptic page for flight controls on the MFD and immediately noticed that one of the left inboard spoilers was indicating full up. At this point, the captain asked ATC for a level off and to stay close to den airport. The captain sent a message to the maintenance and dispatch, reporting the situation. He then informed the flight attendants and the passengers that they would be returning to den for a normal landing. The crew advised ATC that they would be returning to den to which ATC advised that they would be declaring an emergency on their behalf. After declaring an emergency, the captain loaded the new approach, sent for and loaded the landing data, and performed a waant brief. The aircraft landed normally and taxied back to the gate for normal deplaning. During the post flight inspection, it was noted that the left outboard tire had delaminated. Airport ops found the pieces of the tire on the departure runway. The tire tread failure caused minor aircraft damage to the lower left MLG door and underwing access panel. After inspection, it also was found that parts of the tire had impacted and damaged the fire extinguisher discharge tube and moved the tube to the incorrect position. The ground spoiler sensor bracket was found to have been moved to the full end of its travel when the tube was contacted by tire debris.

As the FAA filed it
