From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

This occurrence was reported to the FAA 48 hours after it happened. An experienced skydiver was fatally injured after manually releasing his main parachute at low altitude. The skydiving event was a recreational, 5 person jump originating and kjyl with the drop zone also at kjyl. The jumpers exited the aircraft 13.5 AGL and all 5 main chutes deployed normally. On approach to the drop zone the fatally injured skydiver's main chute released from his harness and he plummeted into a tree line. His reserve chute did not deploy. Post occurrence inspection of the harness revealed that the rsl lanyard was disconnected, preventing his reserve chute from being deployed by the release of his main chute, and the manual reserve chute deployment handle was not pulled. The aad did not activate. The aad was analyzed by the manufacturer and it was determined that the aad did not activate because the required activation parameters were not met.

As the FAA filed it
