From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

This occurrence was referred to our office for investigation: on 4/9/2026 I called a person who did not prove to be the PIC, but gave me the pics's contact information, I emailed the pbr to the PIC. On 4/28/26 I interviewed the PIC about the incident where the aircraft impacted the top of trees on departure to the sw out of vkx, but landed safely. It was determined that the root cause analysis: was that PIC had not completed a w & b or takeoff performance calculations prior to takeoff, as required by 14 CFR 91.103, which caused the FAA to require 44709 reexamination of commercial pilot acs area of operation 1 task f, and flight instructor acs area of operation ii task f. reexamination was completed on 5/14/2026 result was pass.
