From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

He (pilot) taxied out after fueling and performed all the required pre takeoff checks, he started a normal takeoff roll and around 80 - 82 knots, halfway down the runway he felt like he was not accelerating properly. The aircraft briefly broke ground and he selected the gear up but when he realized the aircraft did not appear to have the performance to climb, he attempted to lower the gear back to the down position, all three landing gear came down but it was only enough time for the main landing gear to lock the nosewheel did not, it collapsed and the aircraft came to a rest on the nose, he checked for smoke or fire and shut off the fuel mixture, the prop and throttle remained forward.
