From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

At 1400L, according to the pilot statements, the aircraft was scheduled for dual instruction with a certified flight instructor (CFI). A thorough preflight and walk-around inspection of the aircraft was conducted in accordance with the checklist, and no issues were observed. Prior to taxiing, both the CFI and the trainee verified the functionality of the brakes, which were operating normally.

After completing the checklist, the trainee obtained clearance from cps ground to taxi to runway 30L via taxiway bravo. The trainee stopped at the hold short line without incident and awaited further instructions while cps ground coordinated with a departing jet. Upon receiving clearance for takeoff, the aircraft began to taxi onto the runway.

At this time, the trainee noticed a failure in the braking system, which prevented him from executing the right turn to align with the runway. He promptly informed the CFI, who also attempted to apply the brakes; however, both sets of brake pedals were nonfunctional. The power was immediately reduced to idle, and the aircraft continued to move forward at approximately 5 mph, eventually coming to a stop on the grass just beyond the left corner of the runway.

The control tower was notified, and emergency personnel were dispatched to the scene. There were no injuries, and no damage was reported to the aircraft or runway fixtures. An inspection revealed that the external brake lines appeared intact, with no brake fluid present on the runway. However, brake fluid was observed inside the cockpit beneath the right rudder pedal.

Subsequently, the right-hand brake master cylinder o-rings were replaced. No additional repairs were required. A review of the aircraft logbooks indicated that the aircraft was airworthy at the time of the incident.

As the FAA filed it
