From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

privacy data omitted Suffered a runway excursion during a simulated engine failure during takeoff roll before VMC at kpim (Harris County, GA). Approximately 1220 EDT (on 06-04-26), during a commercial pilot amel end-of-course test (certification ride/practical test). The flight instructor privacy data omitted simulated an engine failure by retarding the left engine mixture control to the idle position after a full engine power run-up and aircraft initiated a takeoff roll. Due to a delayed response from the student (adam mccollum), combined with inability to intervene flight controls/misapplication of rudder input from the flight instructor, the aircraft exited the runway(RWY-9) and made greater than 180-degree left turn until coming to a complete stop in the grass (left side of the runway), and made physical contact with the PAPI lighting system. Both flight crews secured the aircraft and evacuated afterwards. No injuries reported.

As the FAA filed it

^PRIVACY DATA OMITTED^ SUFFERED A RUNWAY EXCURSION DURING A SIMULATED ENGINE FAILURE DURING TAKEOFF ROLL BEFORE VMC AT KPIM (HARRIS COUNTY, GA). APPROXIMATELY 1220 EDT (ON 06-04-26), DURING A COMMERCIAL PILOT AMEL END-OF-COURSE TEST (CERTIFICATION RIDE/PRACTICAL TEST). THE FLIGHT INSTRUCTOR ^PRIVACY DATA OMITTED^ SIMULATED AN ENGINE FAILURE BY RETARDING THE LEFT ENGINE MIXTURE CONTROL TO THE IDLE POSITION AFTER A FULL ENGINE POWER RUN-UP AND AIRCRAFT INITIATED A TAKEOFF ROLL.
