From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

During a multiengine instructional flight, the flight instructor attempted to simulate a left engine failure on the takeoff roll by pulling the left engine mixture to idle cutoff. Directional control was lost, and a runway excursion resulted in which the aircraft struck PAPI lights. Instead of simulating an engine failure, the flight instructor's decision to pull the left engine mixture to idle cutoff caused an actual engine failure, thereby resulting in a loss of directional control. Further, the aircraft utilized on this instructional flight only had brake pedals on the left side of the aircraft, and as such, the flight instructor - seated in the right seat - was unable to utilize brakes.

As the FAA filed it
