What was found
WHEN PILOT ACTUATED THE FLAP SWITCH BOTH FLAPS DROPPED TO FULL DOWN POSITION IMMEDIATELY AND FLAP SWITCH WAS UNABLE TO MOVE FLAPS FROM THAT POSITION. MAINTENANCE DISCOVERED FLAP ACTUATOR HAD SEPARATED FROM GEARBOX DUE TO LOOSE SET SCREWS. REMOVED AND REPLACED FLAP ACTUATOR WITH NEW.
