MAIN ROTOR SERVO HAD THE NR 2 CRANK BOLT INSTALLED LOOSELY. ON DISASSEMBLY OF THE SERVO, PLASMA COATING WAS DISCOVERED IN ALL INTERNAL LOCATIONS.
