What was found
(AUS) NR 7 MAIN WHEEL INNER BEARING COLLAPSED. INVESTIGATION FOUND THAT DURING THE LAST WHEEL CHANGE, THE INBOARD WHEEL SPACER WAS NOTREFITTED TO THE AXLE DUE TO THE SPACER COMING OFF WITH THE WHEEL. PERSONNEL/MAINTENANCE ERROR.
