AFTER TAKEOFF RAMP REPORTED TO TMC THAT NR 2 ENG WAS LEAKING FUEL ON TAXI OUT OF SMF. AIRCRAFT DIVERTED INTO SFO FOR MTC. MTC IN PROGRESS. FOUND FUEL/OIL HEAT EXCHANGER LEAKING. REPLACED SAME.
