What was found
PASSING 12,000 FT, FLT ATTENDANT REPORTED LOUD NOISE. ALSO, HAD LT SLIDE DOOR EICAS MSG AND OVERHEAD DISCREET EMER DOORS. SLIDE HAD DEPARTED A/C WITH FAILURE OF DOOR. REPLACED DOOR SLIDE AND DAMAGED PANELS.
