What was found
THREADS NOT TREATED WITH LOCK-TITE PRIOR TO RE INSTALLATION INTO THE OUTER COMBUSTION CASE, WHICH RESULTED IN THREAD DAMAGE TO THE CASE ITSELF. THE REMOVED VALVE WAS LOST IN SHIPPING. REPLACED WITH NEW UNIT CORRECTED PROBLEM.
