Installation of an air ambulance conversion kit in accordance with Med-Pac, Inc. Drawings as listed on AML No. SA01452CH issued May 14, 2001, or later FAA approved revision.

1) Compatibility of this design change with previously approved modifications must be determined by the installer.

2) A copy of this Certificate and FAA Approved Model List (AML) No. SA01452CH amended November 19, 2004, or later FAA approved revision, must be maintained as part of the permanent records for the modified aircraft.

3) FAA Approved Airplane Flight Manual Supplement (AFMS) as listed on the STC AML Number SA01452CH is required on board the modified aircraft.

4) Instructions for Continued Airworthiness (ICA) Airworthiness Limitations as listed on the STC AML Number SA01452CH are required as part of this modification.

5) If the holder agrees to permit another person to use this certificate to alter the product, the holder shall give the othe r person written evidence of that permission.
