Installation of Great Lakes product windshield part numbers W-2065, W/G-2065, W/T-2065, W-2065-CM, W/T-2065-CM, or W/G-2065-CM in accordance with Great Lakes Aero Products Supplemental Installation Instructions A-4 and A-4CM, revised October 23, 2006, or later FAA approved revisions.
1) The installer must determine whether this design change is compatible with previously approved modification.
2) If the holder agrees to permit another person to use this certificate to alter product, the holder shall give the other person written evidence of that permission.
