FM1068, Revision A, dated February 16, 1994, is required when this modification and SA7463SW are installed.

FM1067, Revision A, dated February 16, 1994, is required when this modification and SA4546SW are installed.

FM1075, Revision A, dated February 16, 1994, is required when this modification and SA892NW are installed.

Compatibility of this modification with other previously approved modifications must be determined by the installer. If the holder agrees to permit another person to use this certificate to alter the product, the holder shall give the other person written evidence of that permission.
