Installation of Sprag System Kit P/N 74-612-153-1M Revision D, dated 9/17/14, according to 74-612-153-INS Installation Instructions Revision C, dated 9/23/2014 or later FAA-approved revisions.

1. Also eligible for installation of OH-13H/Tomcat Mark 6C, STC SH1878SW modification to H5SW Type Certificate.

2. The installer must determine whether this design change is compatible with previously approved modifications.

3. If the holder agrees to permit another person to use this certificate to alter a product, the holder must give the other pers on written evidence of that permission.

This certificate and the supporting data, which is the basis for approval, shall remain in effect until surrendered, suspended, revoked or a termination date is otherwise established by the A dministrator of the F ederal A viation A dministration.
