Installation of C-4 Cabin Kit manufactured in accordance with Carson Drawing List No. 2 dated 12-7-64 and Conversion Instructions dated 11-15-64, or later FAA approved revision.

1. Approved Rotorcraft Flight Manual Supplementary Revision dated 12/18/64 is required. Approved elevator travel limits.

2. FAA Approved Rotorcraft Flight Manual Supplement, dated October 11, 1974, or later FAA approved revision is required with this installation.

3. The installer must determine whether this design change is compatible with previously approved modifications.

4. If the holder agrees to permit another person to use this certificate to alter a product, the holder must give the other person written evidence of that permission.

This cert ificate and the supporting data, which is the basis for approval, shall remain in effect until surrendered, suspended, revoked or a termination date is otherwise established by the A dministrator of the F ederal A viation A dministration.
