Installation of IVAR camera mount, IVAR 500-3 system, on the MDHI Model 369 series helicopter in accordance with Aris Helicopter Ltd., drawing list 369IVAR500-3US, Revision (B), dated April 11, 1994, or later FAA approved revision.

This approval is limited to the installation on MD Helicopters, Inc. Model 369 series helicopters, in combination with High Skid Gear. Rotorcraft Flight Manual Supplement (RFMS) FMS, IVAR Mount MD 500 Model 369, dated October 14, 1993, or later FAA approved RFMS is required. Compatibility of this modification with other previously approved modifications must be determined by the installer. If the holder agrees to permit another person to use this certificate to alter the product, the holder shall give the other person written evidence of that permission.
