The modification must be maintained in aqcordance with Columbia Helicopters, Inc. Maintenance Manual Supplement Document No.

C07-2/77-20-00, Revision 0, dated March 13, 2006 or later FAA accepted revision.

Limitations and Conditions: Approval of this change is limited to aircraft Serial Numbers 4001, 4003, 4004, 4011, and 4012 only, and requires modification in accordance with Supplemental Type Certificate SH432NW.

Columbia Helicopters Rotorcraft Flight Manual Supplement 94-AVI-0903, dated October 21, 1994, or later FAA approved revision is required. A copy of this certificate and Columbia Helicopters, Inc. Master Drawing List 94-AVI-1004, Revision C, dated June 21, 2006 must be maintained as part of the permanent record of the modified helicopter. The Maintenance Manual Supplement Document No. C07-2/77-20-00, Revision 0, dated March 13, 2006 or later FAA accepted revision must be maintained as part of the helicopter instructions for continued airworthiness.
