FAA approved 10/22/2004, including Master Drawing List (MDL) 101-9055, Revision "_" , FAA approved 8/5/2003 and MDL 231-9006, Revision "" *: FAA approved 6/12/1996, or later FAA approved revisions of these documents.

1. This approval should not be extended to rotorcraft of this model on which other previously approved modifications are incorporated unless it is determined that the interrelationship between this change and any of those other previously approved modifications, including changes in type design, will introduce no adverse effect upon the airworthiness of that rotorcraft.

2. STC Number SR00242DE, multi-purpose floor or STC SR00250DE-D, EMS Interior, is a required installation for this modification. Any other Floor System requires further FAA approval.

3. A copy of this Certificate and FAA Approved Rotorcraft Flight Manual Supplement Number 080-0216, dated 8/7/96 or later FAA approved revision must be maintained as a part of the permanent records for the modified rotorcraft.

4. Instructions for Continued Airworthiness, Report AMMS 050-0216, Rev. A, dated 5/21/99, or later FAA accepted revision is required for this installation.

5. If the holder agrees to permit another person to use this certificate to alter the product, the holder shall give the other person written evidence of that permission.
