Limitations and Conditions: Data pertaining to this modification are considered inadequate for duplication in other aircraft. This approval for change in type design is limited to Erickson Air-Crane Model S-64E helicopter, Serial Numbers S/N 64058 and S/N 641001 only. As a prerequisite to this installation, the aircraft must be equipped with an Erickson Fire Fighting system per STC No. SR00004SE and a Federal Aviation Administration (FAA) approved installation of Erickson Main Transmission P/N 6435-20400-063 and Auxiliary Take-off Hydraulic Pump Pad No. 6435-20501-041. The modified aircraft must be operated in accordance with the appropriate Erickson Air-Crane Rotorcraft Flight Manual Supplement (RFMS), dated October 30, 1997 (S/N 64058), and November 6, 1997 (S/N 641001), or later FAA approved revision. A copy of this Certificate and the appropriate FAA approved RFMS shall be maintained as part of the permanent records for the modified aircraft.
