2. Keystone Helicopter Corp. FAA Approved Rotorcraft Flight Manual Supplement No. FM00761, Revision 1, FAA approved August 3, 2005 or latest FAA approved revision, is required with this modification.

3. Keystone Helicopter Instructions for Continued Airworthiness Artex C406-N HM or C406-1 HM Emergency Locator Transmitter installation Sikorsky Model S-76 A/B/C Aircraft, Report No. IC00761, Rev. A dated July 26, 2005 or latest FAA approved revision, is required with this modification.

1. The installer must determine whether this design change is compatible with previously approved modifications.

2. If the holder agrees to permit another person to use this certificate to alter a product, the holder must give the other person written evidence of that permission.
