1. Installation must be in accordance with Heli Lynx Helicopter Service Installation Manual Report No. IM-PA- 04-011-91-01, Issue 1, Rev. Original, Transport Canada approved February 23, 2005 or later Transport Canada approved revisions.
2. Operation must be in accordance with Heli Lynx Helicopter Service Flight Manual Supplement No. FMS-PA- 04-011-91-01, Issue 1, Rev. Original, Transport Canada approved April 5, 2005 or later Transport Canada approved revisions.

3. Maintenance must be in accord ance with Heli Lynx Helicopter Service Maintenance Manual Supplement Report No. ICA-PA-04-011-91-05-01, Issue 1, Rev. Original, Transport Canada accepted April 5, 2005 or later Transport Canada accepted revisions.
4. Compatibility of this design change with previously approved modifications must be determined by the installer.
5. If the holder agrees to permit another person to use the certificate to alter the product, the holder shall give the other person written evidence of that permission.
