Rear Seat Belt Installation in accordance with Heli-Lynx Helicopter Services Inc. Installation Manual IM-PA-04-014-91-01, Issue 1, Rev. Original, approved April 1, 2005, and the Master Drawing List MDL-PA-04-014-91-01, Issue 1, Rev. Original, approved April 1, 2005 or later Transport Canada approved revisions.

1. Maintenance must be in accordance with Heli-Lynx Maintenance Manual Supplement ICA-PA-04-014-05-01, Issue 1, Revision Original, Transport Canada accepted April 8, 2005 or later Transport Canada accepted revisions.

2. A copy of this certificate and the FAA Approved Model List (AML) No. SR02185NY, issued December 7, 2005, or later FAA approved revisions, must be maintained as part of the permanent records for the modified aircraft.

3. Compatibility of this design change with previously approved modifications must be determined by the installer.

4. If the holder agrees to permit another person to use this certificate to alter the product, the holder shall give the other person written evidence of that permission.
