2. Operation must be in accordance with Heli-Lynx Helicopter Services Flight Manual Supplement Report No. FMS-PA-04-012- 91-01 Issue 2, Rev. Original, Transport Canada approved September 9, 2005, or later Transport Canada approved revisions.
3. Maintenance must be in accordance with Heli-Lynx Helicopter Services Maintenance Manual Supplement Report No. ICA-PA- 04-012-05-01 Issue 2, Rev. B, Transport Canada accepted September 9, 2005, or later Transport Canada accepted revisions.
4. Compatibility of this design change with previously appr oved modifications must be determined by the installer.
5. If the holder agrees to permit another person to use this certificate to alter the product, the holder shall give the other person written evidence of that permission.
