1. Installation of a Lifeport MedDeck System 360-3650 on a Eurocopter AS365N3 in accordance with Keystone Helicopter Installation Drawing List IDL02105 Rev A, dated 09/16/2009.

2. Keystone Helicopter Flight Manual Supplement No. FM02105, Rev. -, dated 09/17/09 or later FAA approved revision is required for this installation.

3. The rotorcraft must be maintained in accordance with Keystone Helicopter Instructions for Continued Airworthiness IC02105 Rev. - dated 04/20/2009 or later FAA accepted revision.

1. The installer must determine whether this design change is compatible with previously approved modifications.

2. If the holder agrees to permit another person to use this certificate to alter a product, the holder must give the other person written evidence of that permission.
