This certificate issued to
Part 29 of the Federal Aviation Regulations.
1. Installation of an AXSYS V9 Camera and Moving Map System in accordance with Heli-One Master Drawing List CHA332-25- 040 MDL, Revision C, dated September 4, 2012, TCCA approved September 12, 2012 or later TCCA approved revision.
1. Maintenance must be in accordance with Heli-One, Instructions for Continued Airworthiness report No. CHA332-25-040 ICA, Revision 6, dated September 4, 2012, TCCA accepted September 11, 2012 or later Transport Canada accepted revision.
2. Operation must be in accordance with Heli-One, Flight Manual Supplement, CHA332-25-040 FMS, Revision 6, dated September 4, 2012, TCCA approved September 12, 2012, or later Transport Canada approved revision.
3. The class IIIb laser is electrically disa bled and the wires are capped and stowed.
4. Laser operations are not F AA approved for civil operations.
5. If the holder agrees to permit another person to use the certificate to alter the product, the holder must give the other person written evidence of that permission.
6. The installer must determine whether this desi gn change is compatible with previously approved modifications.
