1. A copy of this certificate and the FAA Approved Model List (AML) No. SR04548CH must be maintained as part of the permanent records of the modified aircraft.

2. Appareo Systems Rotorcraft Flight Manual Supplement, as listed on AML SR04548CH, is required with this installation.
3. The installer must determine whether this design change is compatible with previously approved modifications.
4. If the holder agrees to permit another person to use this certificate to alter the product, the holder must give the other person written evidence of that permission.
