Modification of the vertical fin spar in accordance with Garlick Master Drawing List GHI 94-07, Revision D, dated February 23, 1998, or later FAA approved revision is required.

1. UH-1B aircraft must be equipped with UH-1C/M tail boom (per TM 55-1520-210-23P-1) to be eligible for this modifications.

2. Instructions for Continued Airworthiness (ICA) for the UH-1B, dated November 25, 1997, FTW AEG acceptance dated July 20, 1998, and the ICA for the UH-1E, UH-1L, TH-L, and HH-1K, dated November 25, 1997, FTW AEG acceptance dated July 20, 1998, or later FAA acceptance is required.

3. Compatibility of this design change with previously approved modifications must be determined by the installer.

4. If the holder agrees to permit another person to use this certificate, the holder shall give the other person written evidence of that permission.

5. A copy of this Certificate and ICA must be maintained as part of the permanent records of the aircraft.
